Health Care Clinic Access

Clients have access to a Federally Qualified Health Care clinic on-site. The Health Care District of Palm Beach County operates a clinic within St. Ann Place Outreach Center. The clinic provides daily access to clients that need health care. The clinic can perform vitals checks, perform certain diagnostic testing, address routine health care needs, refer to specialists, triage emergency situations and coordinate with paramedic personnel.

Physical Mailing Address

Clients can use the address of St. Ann Place Outreach Center as their physical address. This is an essential service as physical mail correspondence is required by some agencies to communicate with clients. 

Legal Assistance

With hundreds of tenured and visiting faculty members, you can be sure that you're getting the very best classroom instruction. Our faculty cares deeply about our institution and many of them are graduates themselves. 

ID Assistance

Clients can meet with an attorney that aids in various administrative matters. Common matters our clients face are identification issues (acquiring birth certificates), obtaining access to their social security benefits, and navigating issues related to their housing.

Document Assistance

Housing or shelter programs require individuals to be “Document Ready”, this means that clients must have a “basic” set of documents for submission to various programs. For example, HUD housing requires applicants to have birth certificates, state-issued IDs, and at least two pieces of mail showing their current address. We assist clients in obtaining and organizing these documents.

Behavioral Health Care

Clients have access to a dedicated, Health Care District licensed behavioral health specialist on site. Clients can request a meeting with the specialist to address issues and follow up with further care if needed. A psychiatric nurse, also provided by the Health Care District, is also available on specific days.

Substance Abuse Care

Clients have access to an onsite outreach coordinator if they need help with substance abuse. Clients are referred to the MAT (Medically Assisted Treatment) clinic if they are struggling with drugs and want help. Transportation is provided if needed. The goal is to eliminate any barriers for someone if they want assistance.    This service is provided by Health Care District of Palm Beach County.

Transitional Shelter and Housing Referrals

The Center collaborates with the City of West Palm Beach Community Housing Department to connect eligible clients with low-income and subsidized housing opportunities. Outreach coordinators from the city are on-site each week to meet and assess clients for potential housing. These coordinators also assess clients for temporary shelter at the Lewis Center. If eligible, they can be sheltered for up to 90 days at the Lewis Center while they work on a more permanent housing solution.
