The Health Care District of Palm Beach County (HCD) has partnered with St. Ann Place since 2021. HCD operates one of its 13 Federally Qualified Health Centers at St. Ann Place. Also known as the St. Ann Clinic, this community health center provides outpatient primary care services to the clients of St. Ann Place, regardless of their ability to pay.
HCD’s community health centers offer a full range of services to optimize patient health. These services include medical, dental, behavioral health, psychiatric care, outpatient substance use disorder treatment, women’s health, pharmacy services, and community resource programs. While not all of these services are provided at the St. Ann Clinic, care is coordinated so that patients can access additional services at other HCD locations or via telemedicine when needed. The most common services provided in the St. Ann Clinic includes adult primary care, wound care, health screenings, mental health services, and medication management.
The St. Ann Clinic is open weekdays from 8 a.m. to 3 p.m. The medical team includes a registered nurse, a nurse practitioner, a licensed clinical social worker, and a recovery coach. The Health Care District of Palm Beach County is a safety net healthcare system established by the voters in 1988. It operates Lakeside Medical Center, a skilled nursing facility, the Trauma Hawk aeromedical program, a trauma agency, ground transportation, a school health program, 13 community health centers, and a specialty benefit program for underinsured residents.
The City’s Department of Housing and Community Development Community Services Division provides funding and direct services and programs for the homeless including permanent housing, supportive housing, rapid rehousing, food assistance, relocation assistance, assessments, referral services, and job placement assistance.

St. Ann Place Outreach Center is a homeless resource center located in West Palm Beach, Florida. St. Ann Place provides homeless services including assisting clients with obtaining identification, showers, laundry services, meals, and mail services. St. Ann Place Outreach Center is organized exclusively for charitable, purposes under section 501(c)(3) of the Internal Revenue Code.
